The fusion of neurotransmitter-filled synaptic vesicles with the plasma membrane requires two classes of molecules – SNAP receptor (SNARE) and Sec1/Munc18 (SM) protein. Our reconstitution studies suggest that the SM protein Munc18-1 promotes the zippering of trans-SNARE complexes and accelerates the kinetics of SNARE-dependent membrane fusion. We demonstrate that this trans-SNARE-regulating function of Munc18-1 plays a central role in synaptic exocytosis in cultured neurons. Mutations that disrupt the trans-SNARE-regulating function of Munc18-1 lead to abrogation of both spontaneous and evoked neurotransmitter release. The MUNC18-1 gene is frequently mutated in Ohtahara Syndrome (OS), one of the severest forms of epilepsy. I will discuss how OS-associated mutations impair the regulatory functions of Munc18-1 in synaptic vesicle fusion. Finally, I will talk about our recent effort to dissect insulin-regulated GLUT4 exocytosis and its implications in glucose homeostasis and type 2 diabetes.
